How It Works: Initial Meeting And Assessment

From the moment you call, we begin considering you and your loved one's specific needs. On the initial call, we ask questions to form a basic needs assessment. After the call, we set up an in-home visit with you and the rest of your family – including the family member who may ultimately benefit from our in-home care. This visit allows us to gather even more information, answer questions and let you understand how we can help. To ensure the highest quality care, we will perform a “Guaranteed Compatibility” assessment as part of our staff matching criteria.

Creating The Care Plan

With New Hope Healthcare Services, you will always know what services are being provided to your loved one. During our initial meeting, our registered nurse will develop a Custom Care Plan that is tailored to your loved one's specific needs (also referred to as a “service plan” or “plan of care”). Our RN will make detailed recommendations as to what kind of services would be beneficial to them. We then arrange a follow-up meeting with you to go over the Custom Care Plan line by line and modify it as you see fit.

Caregiver Matching

Once you approve the Custom Care Plan, we begin our search for the most appropriate caregiver to implement care. Our system of matching caregivers to clients takes numerous factors into consideration: services needed, your loved one's interests, and the personalities of both your loved one and the caregiver. At this level of our servicing, we take into utmost consideration our “Guaranteed Compatibility” assessment that was performed at the initial visit. By looking at the whole situation, we're better able to provide your loved one with the best caregiver possible. Of course, if there are ever any concerns about a specific caregiver, we can search for and provide a replacement in very little time.

Supervision Of Care

Our caregivers keep progressive notes about the care they provide your loved ones. Those notes are kept in the home and in our office and are available anytime.  Our supervising nurse makes regular unannounced home visits to ensure caregivers follow the specifics of the Custom Care Plan.
